All intramural and interscholastic sport team participants in grades 6-8 are required to have physical examinations within 365 days of the first tryout date. The physical exam reports are due to the school nurse one week before the tryouts. New forms were developed by the NJ Department of Education and went into effect on January 1, 2008. One section of the new form is a medical history questionairre which is completed by the parent prior to the physical exam, and it must be reviewed and signed by the examining health care provider. The physical exam report must be on the required new state form which can be obtained from the school nurse, athletic coordinator or front office or downloaded from the NJ Department of Education Link @
http://www.state.nj.us/education/districts/forms.htm or
the NJ Academy Of Pediatrics site @ http://www.aapnj.org/csp/aap/main.csp
Physical exam reports are requested in grade 2, 4, and 7. To keep school health records up to date they are welcomed at each grade level. Forms are available from the school nurse or front office.
